Slaves should be prepared to kneel and prostrate at the feet of their Mistress. This is their proper place.
Once a slave is lying on the floor at the feet of his Mistress She may want to show him that he is no more than a worm to be squashed into the dust. She can do this by pressing down upon him with all Her weight using Her arms or She can sit on him to crush him into the dirt.
Some slaves like to be pressed down by the feet of their Mistress who can walk all over them to trample them. There are risks if She uses too much force and She must be careful not to damage the neck or the back. But She can stand on him with one foot on the back of his neck so he is truly under Her heel, or with one leg and a lot of weight anywhere on his upper back or pelvis. If She is going to walk up and down over him he will need to consent in advance and there is a risk of damage if his bones are weak and Her weight is large.
Even if her weight is small it may be best to start trampling with bare feet to spread the load rather than to be wearing stiletto heels which may puncture the skin or even deeper down into the body.
But to be walked over by one’s Mistress really shows a slave his position in life.
Most trampling is carried out indoors in a room with a smooth floor but there is no reason why it cannot be out of doors as well. If Mistress is out walking with a slave then She does not want Her feet and shoes or boots to be made dirty by a puddle or a patch of mud. The slave can provide the bridge so Her boots stay clean. If there is a patch of nettles she does not want to get stung so the slave can be made to lie down on them, take the stings and protect his Mistress from the nettles as She walks over him
